We have been collaborators with community-based organizations such as Benefits Data Trust and United Way 211 in designing and implementing twenty-six unique benefit programs to address social needs for members across the lifespan and life experience. Offerings include transportation benefit services; home-delivered meals after an inpatient hospital, observation, or skilled nursing facility stay; and a home stabilization program that provides resources to members to resolve unsafe housing issues so that they can remain safely in their homes and avoid a nursing facility placement.
